IMPORTANT INFORMATION

The list of participants must be confirmed at least one (1) month before the course starts, to select and prepare future patient cases.

Most importantly, we must comply with the requirements of the Minister of Health of Colombia who requires us to have the following documentation from all participants:

1. The completed and signed form with the personal information. (you will receive it).
2. Copy of passport (in color).
3. The dental certification (copy of the original).
4. The specialty certification – If you have it – (copy of the original).
5. Curriculum Vitae.
